I start with a 6 x 12 piece of cardstock, score 2 inches in from one side...turn the cardstock, then score the center of the 12 inch width of cardstock...then score 2 and 3/4 inches (2 times) from the center score...You will have approx. 1/2 inch left over on either side...
Of the 1/2 inch end sections, remove the top, 4 inch piece...
Close up view of 4 inch section removed...
After the 4 inch piece is removed...go back and remove a bit more, at an angle...
This piece will fold behind the book...and the angle is important for it to lay flat and line up...
